Our office hours are: Monday - Friday, 9 a.m. - 5 p.m.

The office telephones are answered 24 hours a day, and an answering service is provided during non-office hours for urgent matters. If you have an emergency, please call 911.

When calling to schedule an office appointment, please be prepared to provide the following:

• Patient name
• Birth date
• Type of insurance
• Reason for appointment
• Referring physician's name and contact information

If you are unable to keep an appointment, please kindly notify us at least 24 hours in advance. If a significant delay in our physicians' schedule is anticipated, our staff will make every effort to contact you as soon as possible.

In order to provide you with the best service possible, we ask that you bring the following to your appointment:

• Your insurance card
• Your social security number
• Dose and frequency of any medications you are taking
• Referral forms, if required by your insurance provider
• Recent laboratory work, x-rays, or other imaging studies (MRI, CT, Ultrasound, CDs, Bone Scans, Renal Flow Scans, etc.)
• Be prepared to pay your co-pay at the time of your visit:
• We accept cash, check, Visa or Mastercard
• Be prepared to complete a registration form, new patient form, and signed HIPAA acknowledgement form at the time of your visit. Allow at least 10 minutes to complete these forms. Alternatively, you can print out the forms below, complete them at your convenience, and bring them with you to your appointment:
•  Registration Form
•  New Male Patient History Form
•  New Female Patient History Form
•  HIPAA Acknowledgement Form
• Be prepared to provide a urine specimen for analysis at the time of your visit. If you wish to use the restroom before you are seen by your physician, please notify our office staff so they may assist you.

Fees and Payment

Urology Specialists, P.C. requires that all insurance co-pays and deductibles are paid at the time of service. For your convenience, we accept cash, checks, Mastercard and Visa.

Medical Records

There may be a charge for medical records to be released or to complete FMLA or Disability forms. Since Urology Specialists, P.C. strives to maintain your confidentiality, you will be required to sign a release form to obtain your medical records. Please contact our office with any questions or concerns.

Narcotic Prescriptions and Refills on Weekends

No narcotic prescriptions will be phoned in on the weekends between Friday at 5 p.m. and Monday at 8 a.m. Please make sure all of your refill needs are taken care of during normal business hours.

Urology Specialists, P.C. will assist you with your insurance coverage and paperwork to the best of our ability if you present your current insurance card or information at the time of service. Without this, you will be entered into our system as self-pay and will be required to pay at the time-of-service (before treatment).

Co-pays/deductibles: All co-pays or deductibles required by your insurance plan are collected at the time of service.

Forms of Acceptable Payment: Urology Specialists, P.C., accepts cash, checks or major credit cards. In the event that a check is returned as non-payable, Urology Specialists, P.C., may charge a service fee. If you do not have insurance, we will require payment at the time of service. We are willing to establish a personalized payment schedule if necessary to help you get the care you need.

Referrals/prior authorizations: If your insurance carrier requires a referral from your primary care physician, you must present it at the time of service. If you choose to be seen without the appropriate referral documentation, you agree to be responsible for the charges if they are not covered by your insurance carrier. Patients are responsible to complete prior authorizations or pre-certifications as required by their insurance carriers.
